Children's Mental Health Case Management Services
After becoming involved with Children's Mental Health Case
Management services, each family is assigned a case manager. The
case manager meets with the family at least one time each month, and assists the family in developing a team care plan.
This plan could include various types of therapy, special education
services or changes in a child's school routine, mental health
behavioral aid services, social skills building, and family support.
About Case Managers
Case managers provide advocacy and support
to families, as well as helping to coordinate the services the child
is receiving. They have a flexible schedule, and can meet in your home,
at your job, in the community, or at St. David's. Below are some
of the things case managers can assist families with:
- Health
insurance issues, such as applying for Tefra.
- School
issues, such as arranging school meetings and advocating for an
appropriate school program and IEP. Case managers can also refer for
day treatment programs.
- Referrals for individual and family therapy, as well as medication management.
- Referrals for Residential Treatment programs
- Referrals for mental health behavioral aid services and personal care attendant (PCA) services
- Locating appropriate skill building activities and services in the community
Who Qualifies for Children's Mental Health Case Management
To qualify for Children's Mental Health Case Management Services
through St. David's, the child must live in Hennepin County, be
up to 17 years old, and be diagnosed with a severe emotional
disturbance (SED). The child must also be involved in more than
one system, such as special education, juvenile corrections,
mental health services, and medical assistance.
Funding for Children's Mental Health Case Management
Children's mental health case management
services are paid for through the county or medical assistance.
Subsequent services may be funded by insurance, medical assistance,
or on a sliding fee scale.
