Family Group
Conferencing
Family Group Conferencing involves parents and extended
family together to plan for the protection and safety of
children within the family. Families are assisted in identifying their own
strengths, concerns and resources. Families then use this
information to make well-informed decisions about their
children’s future. Families are eligible for this service
if they are involved with Hennepin County Children, Family and
Adult Services Department or the Family Assessment program.
Youth in
Transition Conferencing
Youth Transition Conferencing
gathers young adults transitioning from
foster care to independence. Youth are assisted in identifying
areas of strength and challenge, as well as available
resources. Meetings focus on planning for the future across a
number of life domains including: housing, employment, physical
and emotional health, connections to community, finances, life
skills and education. Youth are eligible for this service if
they are 14 or older and currently in long-term foster care.

agencies in the Minneapolis area that partner with Hennepin
County in providing Family Group Counseling and Youth in
